On or about September 19, 2022, an employee used a pneumatic nail gun to secure shingles to a roof without wearing appropriate eye protection, exposing the employee to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented. On or about September 19, 2022, two employees were installing shingles to the roof of a residential home under new construction and working approximately 20 feet high without fall protection, exposing the employees to a fall hazard.
