29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a): Boiler Room Catwalk - An employee working from an elevated catwalk was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. The employee was exposed to fall hazards of approximately 9 feet while working from the catwalk, on or about, September 19, 2022.  There is no abatement certification or documentation required for this item.

29 CFR  1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:   (a): Boiler Room - An employee working from an extension ladder was carrying a load that caused him to lose his balance and fall. The employee was using an impact gun while leaning to the side of the ladder when he fell striking his head on the hard ground below, September 19, 2022.  There is no abatement certification or documentation required for this item.
