29 CFR 1926.453(b)(2)(v): A body belt with a lanyard was not attached to the boom or basket when working from an aerial lift: On or about September 21, 2022, on the North side of the project site; an employee was utilizing an aerial lift without their personnel fall arresting equipment attached to the boom or basket at heights over six (6) feet.
