29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. a) Fort Campbell Army Base - Bldg. T2-902: On or about 9/21/2022, a floor depression at a 90 degree angle opening approximately 259" long X 188" wide X 6" deep floor that was not protected to prevent workers from stepping into or tripping in the opening exposing them to a fall to the same level.
