Employees were framing the fourth story of a residential structure without the use of fall protection, thereby exposing employees to fall hazards of approximately 30 feet.

Employees were framing the fourth story of a residential construction while standing on a scaffold platform that was less than 18 inches wide, thereby exposing employees to fall hazards.
