T2-902: On or about 9/21/2022, a floor depression at a 90 degree angle opening approximately 259" long X 188" wide X 6" deep floor that was not protected to prevent workers from stepping into or tripping in the opening exposing them to a fall to the same level.

29 CFR 1926.701(b):All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: a) Fort Campbell, Bldg. T2-902:, concrete pad: On or about September 21, 2022, and at times prior, the employer exposed employees to a safety hazard (impalement), in that caps were not placed on exposed reinforced steel while performing the task of conducting conduit checks that was tied to the exposed reinforced steel.
