On or about September 21, 2022 at 112-46 68th Road, Forest Hills, NY 11375 Employees were observed replacing shingles from a residential rooftop approximately sixteen (16) feet above the lower level without a means of fall protection exposing the employees to fall hazards. ABATEMENT CERTIFICATION IS NOT REQUIRED
