At a temporary worksite located at 2703 Woodland Drive, Orange Park, Florida 32073: On or about September 20, 2022, the employer exposed employees to eye injuries, in that eye protection was not utilized by employees, while using pneumatic nail guns to install roofing shingles.

At a temporary worksite located at 2703 Woodland Drive in Orange Park, Florida 32073: On or about September 20, 2022, the employer exposed eight employees to fall hazards ranging from 6 to 14-foot, in that fall protection was not utilized by employees while installing roof shingles.
