On the roof of a temporary residential worksite, at 2440 Washington Street, Orange Park, Florida 32073: On or about September 20, 2022, the employer exposed six employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to install roofing shingles.

On a 4:12 pitched roof of a temporary residential worksite, at 2440 Washington Street, in Orange Park, Florida 32073: On or about September 20, 2022, the employer exposed six employees to fall hazards ranging from 9 feet 7 inches to 16-foot, 5-inches, in that fall protection was not utilized by employees while installing roof shingles.
