29 CFR  1910.23(d)(2):29 CFR 1910.23(d)(2): The employer did not ensure that the minimum perpendicular distance from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder is 7 inches (18 cm), except for elevator pit ladders, which have a minimum perpendicular distance of 4.5 inches (11 cm):  McAllister Hall Tunnel Entrance:  As required by the referenced standard and 29 CFR 1960.8(b):  On, or about, September 19, 2022, the three rung, fixed ladder that provided access to/egress from the tunnel had obstructions behind the ladder due to the installation of pipes.

29 CFR  1910.23(d)(4):  The employer did not ensure that the side rails of through or sidestep ladders extend at least 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder:   McAllister Hall Tunnel Entrance:  As required by the referenced standard and 29 CFR 1960.8(b):  On, or about, September 19, 2022, the three rung, fixed ladder that provided access to/egress from the tunnel was not provided with ladder extensions, a grab bar or other suitable hand-hold for employees to use for stability while using the ladder.

29 CFR  1910.146(c)(1):  The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:  Sail Loft Unfinished Basement:  As required by the referenced standard and 29 CFR 1960.8(b):  On, or about, September 19, 2022, the unfinished portion on the basement level was contained plumbing and electrical systems and was used for storage.  The space had limited access/egress, contained non-contained asbestos fibers, airborne mold particles/spores, was cluttered and contained tripping hazards, was not provided with lighting and did not appear to meet Life Safety Codes for egress.  The space had not been evaluated under the the Confined Space Identification and Entry Program.  Abatement Note:  In addition to evaluating the space for confined space hazards, the atmospheric hazards created by the presence of asbestos and mold need to be identified and quantified to determine the proper level of respiratory protection to be worn by entrants in the future under a Respiratory Protection Program that includes medical evaluations, fit-testing and training.

29 CFR  1910.303(h)(2)(iii)(A):  Conductors and equipment used on circuits exceeding 600 volts, nominal:  The installations shall be made with metal-enclosed equipment or shall be enclosed in a vault or in an area to which access is controlled by a lock;   McAllister Hall Tunnel Entrance:  As required by the referenced standard and 29 CFR 1960.8(b):  On, or about, September 19, 2022, the door providing access to the tunnel entrance was not kept locked.  The room contained 13,200 volt conductors that although insulated, were not protected from accidental contact by an enclosure, barrier, or fence.

29 CFR  1910.335(b)(1):   Safety signs, safety symbols, or accident prevention tags were not used where necessary to warn employees about electrical hazards which may endanger them:   McAllister Hall Tunnel Entrance:  As required by the referenced standard and 290 CFR 1960.8(b):  On, or about, September 19, 2022, the door providing access to the tunnel entrance was not provided with signage to inform employees that live electrical equipment was located inside of the room.  The room housed 13,200 volt conductors that although insulated, were not protected from accidental contact by an enclosure, barrier or fence.
