29 CFR  1926.100(a):At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during construction operations were not wearing hard hats.   On or about September 21, 2022, one employee was working on the ground without wearing a hard hat and passing material above his head to employees working on the roof, exposing the employee on the ground to struck-by hazards.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about September 21, 2022, two employees were framing a roof of a residential home under construction up to approximately 29 feet high without fall protection, exposing the employees to a fall hazards.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  On or about September 21 2022, one employee was performing framing operations of a roof of a residential home under construction up to approximately 29 feet high without the employer providing fall protection training, exposing the employees to a fall hazards.
