1910.304(b)(3)(i) All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops shall have ground-fault circuit-interrupter protection for personnel. 29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: On or about September 21, 2022, at Patterson Rig # 289, a GFCI receptacle was not provided in the laundry area where the water valve was located and used. This exposed employees to electrical shock and fire hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about September 21, 2022, at Patterson Rig # 289, a strain relief on a black portable power cable (marked 480V) was pulled away from plug exposing inner conductors. Power cable was plugged into an electrical outlet located outside Crew Trailer-A. This exposed employees to electrical shock hazards.

On or about September 21, 2022, at Patterson Rig # 289, the Crew Trailer-B had an opening in a stair rail system that exceeded 19 inches at its least dimension. This condition exposed employees to fall hazards.
