29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about September 21, 2022, an employee was using a pump jack scaffold system while performing siding operations at heights greater than 10 feet without fall protection, exposing the employee to the hazard of falling from height.

On or about September 21, 2022, an employee was using a pump jack scaffold system while performing siding operations without the poles of the scaffold system being braced at the top and bottom of the building structure, exposing the employee to the hazard of falling from height.

29 CFR  1926.501(b)(2)(i):Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:   On or about September 21, 2022, employees were performing roof decking operations at heights of 6 feet and greater from a lower level without the use of a fall protection system, exposing the employees to the hazard of falling from height up to 3-stories high.
