29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: a. 2440 Washington Street, Orange Park, Florida 32073: On or about September 20, 2022, the controlling employer Avenue Roofing, LLC failed to ensure that its sub-contractor Bronces Home Solutions, Inc. protected its employees from eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roofing shingles.
