Employees engaged in grain operations at 16515 County Road 14, Chappell, Nebraska 69129, were exposed slips, trips, and falling hazards in that employees were using a stair well for access and egress to and from the grain office. The hole in the stairwell was not protected from employees tripping or stepping into the hole.

Employees engaged in grain operations at 16515 County Road 14, Chappell, Nebraska 69129, were exposed to fall hazards in that the employer did not ensure the guard rail system located at the grain office in front of the scale was capable of withstanding, without failure, force that employees would apply when using the guard rail system for assistance transitioning up and down the stairwell.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. Employees engaged in maintenance operations at 16515 County Road 14, Chappell, Nebraska 69129, were exposed to amputation hazards in that employees were using a hydraulic press that did not have one or more methods of guarding to protect the operator.
