29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 paragraph (b) or (c): On or about September 23, 2022, at a construction site, an employee was welding on water line casings inside of an excavation without adequate protection from cave-ins. G&A Boring Directional, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1545886, citation number 1 item number 3a and was affirmed as a final order on April 11, 2022, with respect to a workplace located at the corner of Kuykendahl Rd. & West Rayford Rd., Tomball, TX 7737.
