In the warehouse at 74 Moylan Lane. Agawam, MA, 01001: On September 23, 2022, and at times prior: employees who operated powered industrial trucks were not trained on the hazards of carbon monoxide or that poor vehicle maintenance could lead to a buildup of carbon monoxide. On August 9, 2022 and September 20, 2022, the Agawam Fire Department responded to carbon monoxide alarms in the facility and powered industrial truck emissions were not investigated as a carbon monoxide source.
