29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.   A)     In the area of the facility where feed was created, there was a John E. Smith and Evans Mixer (buffalo mixer) that had its cover removed for cleaning, exposing a series of rotating blades.  The employee performing the cleaning was authorized to lockout the device, however while the power switch was in the off position, it was not locked out and left unattended.

29 CFR  1910.213(c)(1): Each circular hand-fed ripsaw was not guarded by a hood that completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.   A)     In the maintenance shop, there was a Craftsman table saw that did not have a hood guard installed over the blade.  Employees using the saw are exposed to the portion of the blade that above the material being cut.

29 CFR  1910.213(c)(2): Each hand-fed circular ripsaw was not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator:  A)  In the maintenance shop, there was a Craftsman table saw that did not have a spreader installed to make sure that material would not squeeze the blade and cause the material being cut to be thrown back at the operator.

29 CFR  1910.213(c)(3): Each hand-fed circular ripsaw was not provided with non-kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator.    A)     In the maintenance shop, there was a Craftsman table saw that did not have a spreader with non-kickback fingers installed to make sure the operator was not struck with the material being cut if the saw blade threw the material back at them.

A)     In the nutrition and the genetics QET lab there were fume hoods that were not evaluated to make sure they could maintain a sufficient level of air flow as required by the Chemical Hygiene Plan.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure:  A)     In the selective breeding building, there was a flexible extension cord that was plugged into a wall outlet and run up through the ceiling trusses, and was used on the other end of the room as a pendant power cord.  Fish and Wildlife Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1615871, citation number 1, item number 1 and was affirmed as a final order on 9/26/2022, with respect to a workplace located at Lacey, WA.

29 CFR  1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  A) In the maintenance shop, there was a gallon jug containing a light colored liquid, and the container was not labeled, tagged, or marked as to its contents.
