Bulk Molding Compounds, Inc. - Perrysburg, Ohio: On or about September 24, 2022, the employer did not ensure employees who operated the LeBlond Machine Tool Co. lathe to machine an aluminum shaft were protected from the rotating work piece, by implementing effective machine guarding. b. Bulk Molding Compounds, Inc. - Perrysburg, Ohio: On or about September 23, 2022, the employer did not ensure employees who operated the LeBlond Machine Tool Co. lathe to rewind plastic, were protected from the rotating spools, by implementing effective machine guarding. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

The training program did not include: accessing the regulatory text, explanation of symptoms of bloodborne disease, an explanation and copy of a written plan, information on the hepatitis B vaccination, procedures in the event of exposure, and information on post-exposure evaluation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
