29 CFR 1926.501(b)(15): The employer did not ensure that the employees on a walking/working surface 6 feet (1.8 meters) or more above lower levels, were protected from falling by a guardrail system, safety net system, or personal fall arrest system. On or about August 8, 2022, three employees were exposed to fall hazards greater than 25 feet while working on a commercial construction site in Normal, Illinois and were not protected from falling by a guardrail system, safety net system, or personal fall arrest system.
