29 CFR 1910.178(l)(6): Certification. The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. 1 Lackey Dam Road, Douglas, MA: On or about September 16, 2022, the employer had not maintained a written certification for all employees that were operating powered industrial trucks at the time of the September 16, 2022 inspection showing that each operator had been trained and evaluated as required by this subpart.
