29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  (a) On or about August 16, 2022, as the controlling contractor, failed ensure both Altitude Drywall LTD and The Drywall Company LLLP, protected their employees from falls, while working on a mezzanine being installed in a commercial building, with a section, approximately five foot by five foot, missing, resulting in one employee suffering serious injuries resulting from falling through the hole approximately 16 feet to the lower walking working surface.
