29 CFR 1926.850(a):An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. (a) Hatfield Power Plant, 2773 East Ray Furman Highway, Carmichaels, PA 15320, Precipitation Area on or about September 28th, 2022 at approximately 8:15AM employees were not protected from unplanned sudden collapse of duct work while in the process of dismantling. No engineering survey was completed on the duct work.

29 CFR 1926.858(d): Any structural member being dismembered shall not be overstressed. (a) Hatfield Power Plant, 2773 East Ray Furman Highway, Carmichaels, PA 15320, Precipitation Area on or about September 28th, 2022 at approximately 8:15AM employees were not protected from unplanned sudden collapse of duct work while in the process of dismantling. Relief cuts were made in the duct work resulting in the structural steel becoming overstressed and collapsing.

(a) Hatfield Power Plant, 2773 East Ray Furman Highway, Carmichaels, PA 15320, Precipitation Area on or about September 28th, 2022 at approximately 8:15AM employees were not protected from fall hazards while in the process of dismantling. No fall protection was in use by employees working from heights of ~72 feet above ground level while conducting demolition activities.
