29 CFR 1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. a) Machine shop located in the lower level of the facility: On or about September 29, 2022 and times prior, the employer exposed employees to struck-by hazards in that formal training was not provided to employees who operated the WESCO 76" Lift Powered Lift Electric Fork Stacker (Model #PCBFL - 76-25/ Serial #2219541) throughout the machine shop.
