29 CFR 1910.26(d):The employer did not ensure that measures, such as wheel chocks or sand shoes, are used to prevent the transport vehicle (e.g. a truck, semitrailer, trailer, or rail car) on which a dockboard is placed, from moving while employees are on the dockboard. a) Jobsite located at 3820 North 2nd Street, Philadelphia, PA.- Loading Dock, Door 70: The employer did not ensure that measures were used, such as wheel chocks to prevent a hostler and trailer, on which a dockboard was placed while an employee operating a powered industrial truck to load the trailer. The powered industrial truck fell approximately 4 feet from the dockboard onto the ground level below when the hostler pulled the trailer away from the dock causing the employee to suffer multiple contusions to the head and neck area. Condition was noted to have occurred on or about September 22, 2023.

29 CFR 1910.178(m)(7):Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading: a) Jobsite located at 3820 North 2nd Street, Philadelphia, PA.- Loading Dock, Door 70: The employer did not ensure that measures were used to prevent a hostler and trailer from moving while an employee operating a powered industrial truck was loading a trailer.
