29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions, and openings through which conductors entered were be effectively closed. a) Jobsite located at 6700 Essington Ave., Philadelphia, PA 19153: The employer did not ensure that the charging cables of the Kodiak Charger were protected from abrasions as the cables were resting against the edges of the cable opening. This created conditions where continued use could cause wear on the cable sheathing exposing employees handling the charger to potential shock hazards. Condition was observed on or about September 30, 2022.

29 CFR 1910.305(g)(2)(iii):Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) Jobsite located at 6700 Essington Ave., Philadelphia, PA 19153 : The employer did not ensure that all of the connectors on the charging cables of their Crown powered industrial trucks were provided with strain relief to prevent contact with the conductors. The sheath by the connectors were pulled pack thereby uncovering the conductors exposing employees to electrical shock hazards. b) Jobsite located at 6700 Essington Ave., Philadelphia, PA 19153 : The employer did not ensure that all of the connectors on the cables of their battery charging stations were provided with strain relief to prevent contact with the conductors. The sheath by the connectors were pulled pack thereby uncovering the conductors exposing employees to electrical shock hazards. Condition was observed on or about September 30, 2022.
