29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) The north roof on the north side of the building; On or before November 27, 2022, the employer did not ensure employees were protected from falls to a lower level by the use of a fall protection system.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a) On the roof of the west side of the building: Employees were exposed to falls of 9 feet to the ground because the 50-foot vertical lifelines used had enough slack to allow for the employees to fall over the edge of the roof and impact the ground below.
