29 CFR  1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers.  In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:       (a) Inside the business:  On or about 10/3/2022, employees were not protected from shock hazards while working in close proximity to energized electrical circuits, such as uncovered and dismounted junction and switch boxes.

29 CFR  1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:  (a) At the worksite:  On or about 10/3/2022, an employee was exposed to fall hazards while standing on an exterior open wall section that was under construction at a height of approximately 8 feet above the lower level, without any means of fall protection.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) At the worksite:  On or about 10/3/2022, the employer did not provide a training program for employees who were exposed to fall hazards, which would have allowed the employees to recognize the hazards and the procedures to minimize their exposure.
