Employees did not receive the training necessary to recognize the hazardous energy sources and how and when to secure the electric energy sources for machinery such as but not limited to the pinch roller (SN 205008) in the facility.   b) On or about 10/06/22, the employer did not provide the operators of the glue roller/adhesive spreader (200473) training as authorized employees, in that employees were wiping down the bottom roller while the machine was energized. Employees did not receive the training necessary to recognize the hazardous energy sources and how and when to secure the electric energy sources for machinery such as but not limited to the glue roller/adhesive spreader (SN 200473) in the facility.   c) On or about 10/06/22, the employer did not provide the operators of the pinch roller (SN 205008) training as authorized employees, in that employees were required to wipe down the bottom roller while the pinch roller was energized. Employees did not receive the training necessary to recognize the hazardous energy sources and how and when to secure the electric energy sources for machinery such as but not limited to the pinch roller (SN 205008) in the facility.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (1) Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. (2) Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. (3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). (4) Lockout or tagout device application. (4)(i) Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. (d)(6) Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.   a) On or about 09/23/22, the employer did not ensure that employees applying a protective plastic wrap on the rollers for a porcelain pinch roller, SN 205008, performed elements (d1-d4, d6) of the requirements to ensure all energy sources were secured which resulted in employees that were exposed to an ingoing nip point (caught in hazards.).  b) On or about 10/06/22, the employer did not ensure that employees cleaning an energized glue roller, SN 200473, performed elements (d1-d4, d6) of the requirements to ensure all energy sources were secured, in that employees were exposed to ingoing nip points from rotating parts while wiping the machine with a rag.  c) On or about 10/06/22, the employer did not ensure that employees wiping down the rollers on an energized pinch roller, SN 205008, performed elements (d1-d4, d6) of the requirements to ensure all energy sources were secure, in that employees were exposed to crushing hazards (rollers are capable of closing) and ingoing nip points (rollers are capable of spinning in either direction) (caught in hazards).
