On or about October 04, 2022, at the previously addressed job site, an employee was observed working from a JLG Skytrak  telehandler, with a scaffold platform raised in the air on the forklifts tines of the JLG Skytrak and was used to lift an employee above 15 feet into the air to perform window installation operations creating exposure to fall hazards.
