29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Warehouse, Bay 6 - On or about October 4, 2022,  the employer failed to ensure that an energy control procedure was developed and documented for the control of hazardous energy when employees are engaged in service and/or maintenance tasks, including blade changes on the Tsune TK5C-101GL Fully Automatic Circular Saw, Serial No. 01716.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.) Warehouse, Bay 6 - On or about September 23, 2022, and dates prior to,  authorized employees performed service and maintenance on the Burrmaster, Machine No. 3569.  The employer did not ensure that periodic inspections of the energy control procedures were performed at least annually as required by the standard.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  (a) Warehouse, Bay 6 - On or about October 4, 2022,  an employee performed a service and maintenance task by changing the blade on the Tsune TK5C-101GL Circular Saw, Serial No. 01716.  The employer failed to train each authorized employee performing service and/or maintenance on machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a.) Warehouse, Bay 6 - On or about September 23, 2022, an employee sustained a left hand injury including a broken left pinky and three degloved fingers when the employee's left hand was pulled into the Burrmaster, Machine No. 3569 while manually deburring metal product.  The employer failed to provide effective guarding to protect employees working at or near the machine.
