29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.     On or about October 4, 2022, employees reinstalling shingles on the roof of a multi-story commercial building during reroofing, at a height greater than six feet, were not protected from falls to the lower level.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about October 4, 2022, employees shingling a multi-level commercial building while using pneumatic nail machines were doing so without the use of eye protection exposing the employees to eye or face hazards from flying particles.
