29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about October 5, 2022, employees work from jack scaffolds without fall protection systems while installing cornice material on the exterior of a residential home. The employees were exposed to a fall hazard of approximately 13-feet to the ground below.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about October 5, 2022, employees using a Makita skill saw that had the outer sheathing pulled away exposing internal wiring were exposed to electrical shock hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On or about October 5, 2022, the construction site, the employer did not develop or provide fall protection training to the employees engaged in cornice work to recognize and eliminate hazards such as fall hazards.
