On or about October 5, 2022 at the address of 1621 Bancroft St. Toledo, OH 43606: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

On or about October 5, 2022 at the address of 1621 Bancroft St. Toledo, OH 43606: The employer did not ensure that employees were protected from head injury hazards by protective hardhat while working on the ground below roof line as employees above actively removed old roofing materials. Employees operating in this condition were exposed to struck by injuries from falling/flying objects such as tools and roofing materials.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a)	Albert Peake dba Peake Property Services LLC. On or about October 5, 2022 at the address of 1621 Bancroft St. Toledo, OH 43606: The employer did not ensure employees using two aluminum extension ladders had their side rails extended at least three feet above the landing surface. Four employees performing roofing work activities were exposed to fall hazards while using ladders to access/exit roof on a two-story residential structure with a height of  10ft - 25ft from ground below.

On or about October 5, 2022 at the address of 1621 Bancroft St. Toledo, OH 43606:The employer did not provide a training program for each employee potentially exposed to fall hazards while using two aluminum extension ladders to access a two-story residential roof structure. A training program would enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards.
