29 CFR 1926.452(w)(9): Caster stems and wheel stems were not pinned or otherwise secured in scaffold legs or adjustment screws. Exercise Room: On or about October 5, 2022, the plumber used a baker's scaffold with one wheel stem unpinned and unsecured when he fell from a working platform 63 inches from the ground and sustained head injuries.

29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work: Exercise Room: On or about October 5, 2022, the plumber who fell approximately five feet from a bakers scaffold and sustained significant injuries was not trained in all the hazards associated with the scaffold work.
