29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.   On or about October 5, 2022, and at times prior thereto, employees were framing a new home under construction without the use of hard hats while work was occurring directly above, exposing the employees to the hazard of being struck-by falling debris, tools and materials.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about October 5, 2022, and at times prior thereto, employees were framing a new residential home under construction using a pneumatic nail gun and skill saw without eye protection, exposing the employees to the hazard of being struck-by flying projectiles or debris.

29 CFR  1926.501(b)(13):Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented.    On or about October 5, 2022, and at times prior thereto, employees were framing a new residential home under construction without the use of a fall protection system, exposing the employees to the hazard of falling approximately 35-feet to the ground below.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step:  On or about October 5, 2022, and at times prior thereto, employees were framing a new home under construction using a four foot ladder to climb to the second level the home under construction.  Directly exposing the employees to the hazard of falling from the ladder due to the misuse.
