29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) At the White City OR, VA building Number 234, located at 8495 Crate Lake Hwy OR, on 09-16-2022, a chemical storage room where employees handle Category 1 Serious Eye Damage, including but not limited to NM-50, ND-150 and DEOX, did not have an eye wash / emergency shower immediately available for quick drenching or flushing of the eyes and body as the was located behind a door that could be locked closed. In the event of an exposure and/or spill, the employee's access could be delayed due to obstructions found in the path to the eye wash / emergency shower unit, resulting in the aggravation of eye or skin chemical injuries, resulting in severe eye damage and/or skin chemical burns.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) At the White City OR, VA building Number 234, located at 8495 Crate Lake Hwy OR, on 09-16-2022, a multiple adaptor with a missing ground pin was used to connect a three-wire grounded plug, with three prongs of a "porta-a-cool" portable evaporative unit to a two-wire ungrounded receptacle. The employer failed to ensure that grounded electrical equipment was properly plugged into a compatible electrical service circuit, so in case of a fault, employees were not injured as the result of exposure to electrical energy.

Employees could be exposed to fire and smoke in the event of electrical fire due to overloading of the cord or connections of the surge-protector/power-strip. b) At the White City OR, VA building Number 235, located at 8495 Crate Lake Hwy OR, on 09-16-2022, a Leviton surge-protectors/power-strips used to power electrical chargers were attached to building's wood surfaces/walls. Operators of a chain saw with missing chain catcher are exposed to struck-by injuries and lacerations if case of a chain breakages.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead: a) At the White City OR, VA Police Station, located at 8495 Crater Lake Hwy OR, on 09-16-2022, the employer failed to ensure that work surfaces in the armory/evidence room, were reasonable free of Lead contamination. A ghost wipe sample of 100 square centimeter surface of the desk used by police officers to clean guns after shooting practice, recorded the presence of 383.96 micrograms of inorganic Lead. Employees exposed to inorganic Lead contaminated surfaces and accidentally ingest it and suffer symptoms of Lead poisoning. b) At the White City OR, VA Police Station, located at 8495 Crater Lake Hwy OR, on 09-16-2022, the employer failed to ensure that work surfaces in the armory/evidence room, were reasonable free of Lead contamination. A ghost wipe sample of 100 square centimeter surface of a storage shelve where police officers fill logs and other paperwork, recorded the presence of 11.85 micrograms of inorganic Lead. Employees exposed to inorganic Lead contaminated surfaces and accidentally ingest it and suffer symptoms of Lead poisoning.

Failure to provide adequate information on the identity of the chemical in the container and hazards it presented to employees, could expose employees handling the chemical to injury or toxicity.
