29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 6, 2022, on the east side of the construction site, employees installing trusses worked without fall protection, were expose to fall hazards approximately 10-feet to the lower deck.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided. On or about October 6, 2022, the employer did not provide an extension ladder where employees could access and egress the second level of the commercial building, exposing them to potential fall hazards.
