Employees working in this condition were exposed up to a 19 ft. fall to the ground below.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed a) Euceda Construction, LLC, on or about October 6, 2022, at the address of 1020 Pine Valley Lane, Toledo, OH: The employer did not ensure the side rails of one portable aluminum extension ladder was extended at least three feet above the landing surface. Three employees were exposed to approximately 19 feet fall to the ground below while using the ladder to transition to the roof level.
