(a) Jobsite, I 376 Eastbound under the Oakland offramp in Pittsburgh, Pennsylvania: On or about October 7, 2022 at approximately 3:00 am, an aerial lift bucket extended over an open traffic lane and the area below the bucket was not closed to traffic and properly barricaded to protect workers. The bucket was struck by a tractor trailer driving through the open traffic lane, resulting in fatal injuries to the employee working from the elevated bucket stringing steel cables.
