29 CFR 1926.652(e)(1)(i): Members of support systems were not securely connected together to prevent sliding, falling, kickouts, or other predictable failure: (a)Backhaus Excavation & Underground, LLC. at 7400 East Oak Grove Trail, Sioux Falls, SD 57106: On or about and at times prior to October 6, 2022, the exposing employer did not ensure that protective systems installed in the trench excavation were installed securely to prevent sliding, falling, kickouts, or predictable failures, in that an upper spreader bar pin was not installed to ensure structural stability of the protective system. This condition exposed employees to struck-by hazards associated with support system failure.

This condition exposed employees to struck-by hazards associated with loose falling rock or soil.

This condition exposed employees to hazards associated with the use of damaged extension ladders.
