29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about October 3, 2022, at the above addressed jobsite, employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a. On or about October 3, 2022, at the above addressed jobsite, employees improperly used an A-Frame  ladder by placing it on the garage roof, leaning it to the second story roof, and using it to gain access to the second story roof, thereby exposing the employees to the hazards associated with falls of nine (9) feet.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about October 3, 2022, at the above addressed jobsite, employees performing residential roofing operations, nineteen (19) feet eight (8) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  Vega Installation Inc., was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1608965, citation number 1, item number 1and was affirmed as a final order on November 29, 2022, with respect to a workplace located at 10051, 10049 Court F, Cambridge Dr., Mokena, IL 60448.
