This was most recently documented on October 7, 2022 at 1314 8th Street, Beatrice, Nebraska when employees were observed working on a residential structure with a roof. The employees where installing shingles while working at a height of approximately 10 feet 8 inches measured from the ground to eave without fall protection.

This was most recently documented on October 7, 2022 at a residential construction jobsite located at 1314 8th Street, Beatrice, Nebraska. Employees were using a ladder to access the roof that was not extended over the upper landing surface at least three feet.
