29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about October 6, 2022, and at times prior thereto, employees were working inside an excavation to install a tie-in to a natural gas pipeline without the spoil pile at least two feet away from the edge, exposing the employees to the hazard of being struck-by falling rocks, debris and materials.
