As of April 12, 2023, the employer had not provided abatement documentations for Citation 2,  items 1a, 1b and 1c, contained in inspection 1627268, which was issued on December 8, 2022 and has a final order date of January 5, 2023 with respect to  violations at 130 Greenacre Ave., Longmeadow, MA.

29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  130 Greenacre Ave., Longmeadow: On or about October 7, 2022, the employer had not ensured that employee(s) using  portable extension ladders to access an upper level and were installed in manner in which the working angle did not exceed a ratio of 4:1 or one quarter the working length of the ladder. The ladder was set up exceeded a 4:3 ratio  exposing employees to ladder displacement and fall from height hazards.

29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws:  130 Greenacre Ave., Longmeadow, MA: On or about October 7, 2022, the employees at this work site were using a 7 1/4 inch DeWalt Circular saw powered by a yellow extension cord which was not provided with strain relief which would prevent pull from being directly transmitted to joints or terminal screws.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:  130 Greenacre Ave., Longmeadow, MA: On or about October 7, 2022: Employees were exposed to fall hazards are part of their assigned duties and the employer had not trained them prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures.  Eliezer J Perez dba EJ's Home Improvements  was previously cited for a violation of this occupational safety and health standard or its equivalent standard  29 CFR 1926.503(a)(1) , which was contained in OSHA inspection number 1583366, citation number 1, item number 1b and was affirmed as a final order on 08/26/2022, with respect to a workplace located at 21 Fairfield Terrace, Longmeadow Ma.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
