1910.147(c)(6)(ii)  The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  On or about September 23, 2022, the employer did not certify that a periodic inspection was conducted on the energy control procedure for Crossover Conveyor #2. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   On or about September 23, 2022, the employer did not ensure that employees who performed service and maintenance tasks on the Crossover Conveyor #2 were adequately trained on the means and methods needed to effectively isolate and control hazardous energy sources. Employees were exposed to pinch and nip point hazards of equipment in operation which could result in caught-in, crushing and amputation injuries while performing the clean-out/kernel clean process.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  On or about September 23, 2022, the employer did not ensure that employees implemented energy control application steps prior to allowing them to perform the clean-out/kernel clean process on Crossover Conveyor #2 at the West Husking Beds (Green Corn). The conveyors were not shut down or turned off prior to employees engaging in this servicing and maintenance activity, exposing them to pinch/nip point hazards and amputation injuries.   As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
