29 CFR  1904.41(a)(2):  The establishment had 20 or more but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified as an industry listed in appendix A to subpart E of this part, but did not electronically submit information from OSHA 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee.  The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  ESTABLISHMENT:  On or about March 29, 2023, the employer had not submitted the injury tracking application to Federal OSHA for the year 2022.

29 CFR  1910.157(e)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  WAREHOUSE:  On/or about October 11, 2022, the employees were exposed to the following unmounted and blocked portable fire extinguishers.  1.   Shipping and Receiving Door Area:  The ABC fire extinguisher was unmounted and not accessible (located near pallets containing product referred to as Flavor 59 tea, supplier SNA Tea), broken hook on the column to  hang the fire extinguisher.  2.  Shipping and Receiving to Bay 3:  The ABC fire extinguisher, B456 model, was unmounted.  3.  Bay 3:  The ABC fire extinguisher was  located on a work station on top of pallets between work station on Tea/liquor pallets and unmounted.

29 CFR  1910.178(p)(1):Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s):  WAREHOUSE:  On/or about October 11, 2022, the employees were exposed to the following powered industrial trucks (PITs) containing defects and were not removed from service:  1.  Shipping/Receiving Door Area/First Floor:       a.	N3 Toyota model 8FGC25, was not equipped with a front light on the right side and area was taped.   2.   Bay 2: Second Floor       a.	N1 Toyota model SFGC8020 was not equipped with back up lights.       b.	Toyota 31, model 7PGC6U20 was not equipped with backup light on the right side.
