29 CFR 1910.30(a)(1):(a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. a. Located at FST Logistics, 3280 South Park Place, Grove City, Ohio 43123: On or about October 10th, 2022, the employer did not ensure that employees were trained in the proper use of personal fall protective systems, such as but not limited to, proper adjustment of harness straps and the dorsal D-ring attachment point at shoulder level; and proper attachment of the lanyard directly to the attachment point while operating Toyota Electric Order Selector Forklifts in aisles 41/CP-08 and CP-12 at elevations approximately 12 feet from the floor.

29 CFR 1910.157(c)(1):The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. a. Located at FST Logistics, 3280 South Park Place, Grove City, Ohio 43123: On or about October 10th, 2022, the employer did not ensure that the fire extinguisher placed on the floor in aisle 53, bay 26, was mounted in its designated location on the racking upright.
