29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:   a) Shipping Department- On or about October 12, 2022, the employer did not ensure that the energy control procedures that were established, were specific to the equipment such as the Old Lantech Stretch Wrapper, and clearly outlined the following: a specific statement of the intended use of the procedure (servicing, maintenance, and/or un-jamming), procedural steps for shutting down, isolating and securing the machine to control hazardous energy (electrical, and pneumatic), procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and specific requirements for testing the machine to determine and verify the effectiveness of the lockout devices, tagout devices and other energy control measures.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):   a) Shipping Department- On or about October 12, 2022, employees were exposed to the unexpected energization of the Old Lantech Stretch Wrapper. During changing of the rolls and accessing the wrapping area the pneumatic energy was not turned off.      The employer failed to implement energy control application steps as the machines were not isolated from all energy sources [per the 1910.147(d)(3) requirements].  As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.   b) Consumer Department- On October 13, 2022, employees were exposed to the unexpected energization of the C002 Lid Flipper and the C003 Lid Flipper during unjamming of the lids.    The employer failed to implement energy control application steps as the machines were not shut down or turned off to perform adjustments [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR  1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s):  a) Press Department- On October 25, 2022, the employer failed to provide guards on mechanical power presses, such as Press #55, which prevented employees from being able to reach through, over, under and around the guards to come into contact with the point of operation.   b) Press Department- On October 13, 2022, the employer failed to provide guards on mechanical power presses, such as Press #127, which prevented employees from being able to reach through, over, under and around the guards to come into contact with the point of operation.   c) Consumer Department- On October 25, 2022, the employer failed to provide guards on mechanical power presses, such as Press #134, which prevented employees from being able to reach through, over, under and around the guards to come into contact with the point of operation. The gang box had been wired to a flexible cord to provide additional outlets in the style of an extension cord for the Longford Motor Control.
