Shaker Area (near metal grate flooring): On about 10/3/2022 and at times prior, the employer exposed employees to trip hazards in that the metal grate flooring was not leveled with a three-inch lift when employees were required to perform task such as enter or exit out of the skid to retrieve material in sand.
