29 CFR  1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  Shaker Area (near metal grate flooring): On about 10/3/2022 and at times prior, the employer exposed employees to trip hazards in that the metal grate flooring was not leveled with a three-inch lift when employees were required to perform task such as enter or exit out of the skid to retrieve material in sand.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  a) Rear of Facility (near Blasting Area & Quality Control Area): On or about 10/3/2022, and at times prior, the employer exposed employees to entrapment and fire hazards, in that the exit route was obstructed with equipment as well as overgrown yard and litter while employees are performing tasks such as quality control and welding.
